Practice Policies

Your clear understanding of Stellar Medical Aesthetics’ (the “Practice”) Practice Policies is important to our professional relationship.

You are electing to undergo treatment that is not covered by insurance.   Payment is expected at the time of service.  We accept cash, check and major credit cards as forms of payment.  

Unfortunately, should our billing office fail to collect the balance on a patient's account after 60 days, and prior arrangements for payment have not been established, the Practice may then place the account into collection status.  In the event that the account is placed in collection status, an administrative fee will be added to the account balance, and the patient will be responsible for all attorneys’ fees and costs incurred in collecting any outstanding balance.

Cancellation/No Show Policy. We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family.  However, when a patient does not show up, another patient loses the opportunity to be seen.  Therefore, we request that you notify the office of a cancellation at least 24 hours in advance.  Clients who do not notify the office of a cancellation at least 24 hours in advance will be charged a $100 cancellation fee.  Clients who do not notify the office of a cancellation and do not show for their appointment will be charged 50% of the fee for the services booked.  If you arrive more than 15 minutes late for your appointment, it may be rescheduled depending on the volume of patients and the time allotted for your visit. 

The Practice reserves the right to request a deposit to make an appointment. The deposit will be credited to the services rendered at your appointment, and in the event of a no show or cancellation less than 24 hours before your scheduled appointment, the deposit will be applied to cancellation or no show fees, and will NOT be refunded.

Fees for various procedures provided by our practitioners and staff vary.  Upon request, we will provide these fees to you.  We do not offer refunds on services rendered. 

Treatments sold as a “series” may not be divided and used by more than one patient.  If you are unable to use treatments purchased, no refunds will be granted.

No substitutions are made for special offers or promotions.

Skin care products and other items sold by the Practice are not returnable.  All sales are final.  Should you have an allergic reaction to a skin care product, please contact us to discuss.    

Privacy. Stellar Medical Aesthetics is required by law to maintain the privacy of protected health information. At the Practice your privacy will be protected and your medical records will not be shared without permission, except as required by law or as requested by a patient or patient representative or other legally designated acknowledgments.  By signing below, you acknowledge that you have received and reviewed Stellar Medical Aesthetics’ Notice of Privacy Practices.
